Witness statements

How should I file a witness statement?

Start with a title to identify the proceedings and then give your name, address and occupation. If you are giving evidence in a professional or business role, you can give your working address, your position and the name of your employer instead of your home address.

Write your evidence in the first person, for example "I am Managing Director of the company and I have held this position for 3 years.", and number each paragraph.

Include the following statement of truth:

"I confirm that the information contained in this statement is true to the best of my knowledge and belief."
