Section 1

You don’t have to provide a reference, but if you do, we will use it whenever we contact you.

Section 2

You should not fill this in, it is for our use.

Section 3

Enter the full name, address and postcode of the person applying.

Section 4

You must supply a contact address, which we call an address for service, which may be in the European Economic Area, to which all our letters should be sent.

If you appoint someone for example, a patent attorney to deal with your application, give details here.

If you leave this section blank, we will write to you at the address given in section 3.

Section 5

Enter whether the application is made for a medicinal or plant protection product.

Section 6

Enter the active ingredients the certificate will protect.

Section 7

Enter the number, title, expiry date and, if later than the first UK authorization, the date of grant of the basic patent.

Section 8

Enter the number and date of the first UK authorization.

Section 9

If different from the first UK authorization, enter the State, number and date of the first authorization in the EC, plus the identity of the authorized product and the legal provision under which the authorization took place.

Section 10

Enter confirmation of the marketing authorizations, translations and any other information or documents that you are sending with your application.
